What is Sema+Cag?
Sema+Cag is a PEPTYVA research vial that combines two peptides: semaglutide 5 mg and cagrilintide 5 mg. It is sold for laboratory research use only and is not a medicine. Semaglutide is a GLP-1 receptor agonist, and cagrilintide is a long-acting amylin analog.
Novo Nordisk is developing the same pairing as CagriSema, an investigational product. Sema+Cag is not CagriSema. It shares the two active molecules, but it is a separate research material made by PEPTYVA with its own lot documentation.
How is the semaglutide and cagrilintide pairing studied?
The pairing is studied because it engages two hormone pathways at once: GLP-1 and amylin. Controlled studies compare the combination with each component alone to see how the two pathways interact in glycemic and body weight regulation.
- Semaglutide: selective GLP-1 receptor agonist with a lysine side chain linked to a C18 fatty diacid
- Cagrilintide: long-acting amylin analog acylated at the N-terminus with a C20 fatty diacid
- Typical research topics: amylin and incretin receptor pharmacology, appetite signaling, glucose regulation
- Study design feature: factorial comparisons against each single component and placebo
What does the current evidence show?
Controlled human studies exist, but the combination is still investigational. REDEFINE 1 (New England Journal of Medicine, 2025) randomized 3,417 adults with overweight or obesity and without diabetes to the combination, semaglutide alone, cagrilintide alone or placebo for 68 weeks. REDEFINE 2 (New England Journal of Medicine, 2025) enrolled 1,206 adults with type 2 diabetes and overweight or obesity for 68 weeks.
Both phase 3 trials met their primary body weight endpoints versus placebo. Gastrointestinal adverse events, such as nausea, vomiting, diarrhea and constipation, were frequent in both trials and mostly mild to moderate. No direct evidence exists for this specific PEPTYVA vial, as opposed to the sponsor's investigational product.
How does Sema+Cag differ from semaglutide alone?
Semaglutide alone acts only on the GLP-1 receptor and is the active ingredient in approved branded medicines. Sema+Cag adds cagrilintide, an amylin analog, and the combination has no regulatory approval. That difference is why semaglutide carries grade A and Sema+Cag carries grade B on this store.
How is a two-peptide vial documented?
A combination vial needs documentation for each component. The Certificate of Analysis for a Sema+Cag lot should identify both semaglutide (about 4114 g/mol) and cagrilintide (about 4409 g/mol), typically by mass spectrometry, and report purity, typically by HPLC. The lot number and expiry date on the vial link it to that COA.
What is the regulatory status of Sema+Cag?
The combination is investigational. Novo Nordisk filed a New Drug Application for CagriSema with the FDA in December 2025 and, in September 2026, said it expects a decision in the fourth quarter of 2026. Any future approval would belong to the sponsor's branded product and would not extend to this PEPTYVA research vial.
Documentation
Each vial carries a PEPTYVA lot number and expiry date. The Certificate of Analysis for that lot can be requested through our contact page; for a two-component vial it should identify both semaglutide and cagrilintide. See Quality & Documentation for how to read a COA.
Storage and handling
Keep the unopened vial sealed in its original packaging, protected from light and heat, at the storage temperature printed on the PEPTYVA label. Check the strength, lot number and expiry date on arrival, and do not use material past the expiry date shown on the label.

Combination aimed at appetite, satiety and metabolic strategies.
Regulatory status
- United States
- Not FDA-approved. Novo Nordisk submitted a New Drug Application for CagriSema (cagrilintide 2.4 mg and semaglutide 2.4 mg) in December 2025 for weight management in defined adults. In September 2026 the company stated that CagriSema remains investigational and that an FDA decision is expected in Q4 2026. Semaglutide alone is approved only within specific branded products (Ozempic, Rybelsus, Wegovy). No approval covers this PEPTYVA vial.
- Mexico (COFEPRIS)
- The sponsor describes CagriSema as investigational. This PEPTYVA vial is research material, is not a registered medicine in Mexico and is not for human use. Its semaglutide component is, in registered branded form, a prescription medicine in Mexico (COFEPRIS, 2023).
